The movie, based on myths that surround 99 Mansion Nibong Tebal, focuses on the titular home of Tengku Izzudin Shah, who lived there with his wife and son. Unfortunately, his family members all mysteriously died. Before he himself later passed away, he asked for his house to be turned into a hospital. However, the house has now been left abandoned for 30 years. Hisyam, a contractor, offers to do restoration work on it and his offer is accepted by Putera Shah's firm. Together, the two of them, along with Rafael and Camelia, visit the house and set up camp near it for the night. That is when they begin experiencing strange and frightening occurrences at the house.

Joey, the owner of television show "Merungkai Misteri", has been called to Kampung Bonang to investigate the mystery surrounding the gruesome death of Cikgu Hassan. Following his death, the villagers claim that his spirit can be seen wandering the village looking for a new successor. At the same time, his daughter Julia often doesn't feel well now as she is disturbed by supernatural entities. To clear Cikgu Hassan's name, Joey kidnaps Lufty, a man who is unwittily roped into this while looking for a job, to be his crew during the investigation. The mystery deepens when Joey and Lufty, joined by Julia, are disturbed by the spirit Penunggu Busut while on the way to Busut and they lost their way.
